＃家庭組每組最多四人，隊員最少須包括一名14歲以下兒童及一名18歲或以上人士 Each family group could consist of a maximum of 4 people, in which at least 1 participant below the age of 14 and 1 participant with the age of 18 or above should be included

報名費Entry Fees：

報名 ： 填妥報名表連同劃線支票(抬頭請寫”香港野外定向總會”)寄回總會，支票背

Application         面請寫上參加者姓名及賽事名稱

                                 Mail the completed entry form with a crossed cheque payable to "Orienteering Association
                    of  Hong Kong" to our office. Please write down the name of the participant and event name
                                  at the  back of the cheque

獎項 Prizes ： 不設任何獎項  No award will be presented

備註 Remarks:

1. 所有總會公開賽將採用EMIT電子打孔及計時系統。參賽者必須使用有效之EMIT電子控制咭參賽

EMIT Electronic Punching & Timing System will be used in all OAHK Events.
All participants should use the valid EMIT E-cards for the competition

2. 賽事資料於賽事舉行前一星期上載於總會網站 www.oahk.org.hk，各參加者必須自行瀏覽

 Event information shall be uploaded on OAHK website: www.oahk.org.hk one week before the event.
 Participants must visit on their own.

3. 大會不接受傳真報名及期票

Application by fax and post dated cheque will not be accepted

4. 大會已購買公眾責任保險，參加者/家長可考慮自行購買其他保險

 Public Liability Insurance is covered by the Association.
 Participants/Parents are recommended to apply other insurance

5. 你所提供的資料只用作本會與合辦機構的康體活動報名事宜，在遞交申請表後，
 如欲更改或查詢你的個人資料，可與本會職員聯絡

  The information provided by you will only be used for the enrollment of recreation and sports
  activities organized by our Association and co-organizing parties. For correction of or access to
  personal data after submission of this form, please contact the staff of our Association.

6. 大會保留最後修訂之權利
